      Calcaneal (Heel) Osteotomy In Turkey

Calcaneal or heel osteotomy operation is a minimally invasive surgical procedure where the heel bone (calcaneus) is cut and repositioned to correct deformities or misalignments. This procedure is often indicated when there are abnormal arch conditions, such as high or low arches, that affect the overall foot function. It is most often performed on patients with conditions like flatfoot, where the arch of the foot collapses, leading to pain, instability, and difficulty with mobility.

The repair process for a calcaneal osteotomy involves cutting and repositioning the heel bone to correct alignment.
The goal of performing a calcaneal osteotomy is to realign the bone, relieve pain, reduce pressure on certain parts of the foot, and reduce the risk of developing arthritis in the foot joints over time. The technique chosen will depend on the patient’s specific needs, and the four primary types of osteotomies; Evans, Dwyer, Medializing, and Lateralizing.

The Evans osteotomy corrects the flatfoot by lengthening the outside edge of the foot, while the Dwyer osteotomy focuses on lowering the height of abnormally high arches. Medializing osteotomy is used for restoring fallen arches, and Lateralizing osteotomy corrects high arches by shifting the heel bone outward.

Advanced stages of foot conditions requiring calcaneal osteotomy often present with persistent heel pain that limits daily activities and worsens with weight-bearing. Obvious deformities, such as a collapsing arch or a rigidly high arch, often accompanied by visible misalignment of the foot and ankle. Instability and imbalance, making walking or standing difficult, may also be present. Arthritis may also develop due to prolonged misalignment and joint stress.

Who Are The Eligible Candidates For Calcaneal (Heel) Osteotomy?

A calcaneal osteotomy is recommended for individual who meet certain criteria and exhibit specific foot conditions. Eligible candidates include those with:

      • Flatfoot deformity or posterior tibial tendon dysfunction.
      • High arches (cavus foot) causing pain or instability.
      • Misalignment leading to walking difficulties.
      • Chronic heel pain.
      • Failed non-surgical treatment, when physical therapy or orthotics don’t work.

While a calcaneal osteotomy offers significant relief from discomfort and improves mobility, it’s essential to consult your doctor to ensure it’s the right option for you. Non-surgical treatments like rest, ice, immobilization, NSAIDs, or physiotherapy should be tried first as they may provide adequate relief.

How Is Calcaneal (Heel) Osteotomy Performed?

It is usually combined with other reconstructive procedures to address multiple issues within the foot. This could involve procedures to correct soft tissue abnormalities, tendon dysfunction, or joint degeneration in addition to bone realignment. Typically, patients will be given general anaesthesia and depending on the complexity of the surgery, local anesthesia may be used to provide additional pain relief. A calcaneal osteotomy surgery takes about 1 to 2 hours.

      Preparation for the Surgery

Prior to arriving in Turkey, patients should have tried non-surgical treatments such as orthotics, physical therapy, or immobilization.
Patients are advised to send their medical reports like X-rays and MRIs and reports from physical therapy or prior treatments before surgery.
It is important to discuss about any ongoing treatments or medical conditions, such as diabetes, arthritis, as some medications that may need to be adjusted before surgery e.g., blood thinners.

      Types of Anaesthesia

Calcaneal osteotomy surgery is performed under general anaesthesia, which will ensure the patient is completely unconscious during the procedure. However, depending on your condition and surgeon’s preference, regional anesthesia, such as an epidural or spinal block, may be an option to numb the foot and lower leg while keeping you mildly sedated.

General anaesthesia is administered through an intravenous (IV) line placed on your arm or hand. Through this line, medication will be injected that will induce sleep and numbness. Gas anaesthesia may be used in some cases, where you breathe in a special gas through a mask or tube to maintain the sleep-like state.

      Techniques for the Procedure

The surgeon begins by making an incision on the lateral (outer) side of the foot and cut the heel bone (calcaneus) into two sections. The lower section of the heel bone is then shifted to a more aligned position in relation to the rest of the foot to restore proper function and alleviate pain caused by conditions like flatfoot or heel deformities.

Once the bone is realigned, surgical screws are inserted to stabilize the sections. Depending on the deformity, a bone graft may be used as part of the procedure. After the osteotomy is complete, the wound is closed with either stitches or staples, and a dressing is applied to protect the site. A plaster cast is placed around the foot to ensure proper healing and immobilization
