Who Are Eligible Candidates For The In Vitro Fertilization (Ivf) Treatment?

Today, many methods have been developed for couples with fertility problems to have children. In vitro fertilization is recommended for couples and individuals who have not achieved results with these treatments. As is known, in vitro fertilization treatment is applied to people who are diagnosed with infertility and cannot become pregnant naturally.

In vitro fertilization treatment is recommended in the following conditions: Even though couples aged 35 and under have had regular and unprotected intercourse for 1 year; If pregnancy does not occur despite regular and unprotected intercourse for 6 months in women aged 35 and over because the egg reserve decreases, and if pregnancy does not occur in women over the age of 40 despite regular and unprotected intercourse for 3 months.

Other situations where in vitro fertilization treatment is deemed appropriate: Women who have menstrual irregularities, decreased oocyte reserve, have had an ectopic pregnancy, have had ovarian inflammation, have uterine adhesions or closed tubes due to abdominal surgery, have a sexually transmitted disease.

How Is The In Vitro Fertilization (Ivf) Treatment Performed?

In cases where pregnancy does not occur despite regular and unprotected intercourse, couples should first consult a doctor. The doctor will perform some tests to evaluate the couples' suitability for in vitro fertilization treatment.

IVF treatment consists of 6 stages and lasts about 18-20 days.

      PREPARATION

The doctor will request some tests from male and female individuals to determine their suitability for in vitro fertilization treatment.

      TESTS ON FEMALE INDIVIDUALS

Basal hormone tests are applied to understand egg quality and ovarian reserve. These tests are E2, LH, TSH, RRL and in some cases DHEAS and total testosterone tests are requested.
Then, hysterosalpingography is performed by injecting contrast fluid into the uterus to check the uterus and ovaries.
After pregnancy occurs, there are required tests and these are performed to investigate diseases that may be transmitted to the baby. These tests are AIDS (HIV), Hepatitis B, Hepatitis C, Rubella IG G, blood group test, blood clotting level tests and full hemogram.

      TESTS ON MALE INDIVIDUALS

For male individuals, spermiogram is performed. Thus, sperm count, motility and quality are investigated.
Some hormone tests are also requested from men. These tests are PRL, FSH, LH, Total testosterone, Hepatitis B, Hepatitis C, AIDS (HIV), blood group analysis and, when necessary, chromosome analysis.

Stages Of In Vitro Fertilization (Ivf) Treatment

      1st Phase - Oocyte Stimulation

At this stage, an attempt is made to obtain a large number of oocystes by using ovarian stimulating medications. Medications doses are adjusted according to the individual's body mass index (BMI), oocyte reserve and, if they have previously received IVF treatment, the previous medication dose and the collected oocyte.

      2nd Phase – Collecting Oocytes

At this stage, sedation or anesthesia is applied to the individual to prevent discomfort and pain. Oocystes are collected with an ultrasound vaginal probe fitted with a thin hollow needle. The collected oocytes are immediately sent to the laboratory. Individuals may experience cramping and pain after this procedure.

      3rd Phase - Taking A Sperm Sample

At this stage, a sperm sample is taken on the same day as oocytes collection. Sperm sample is taken by masturbation. If there is no sperm in the semen, sperm can be removed surgically. The samples taken are immediately sent to the laboratory.

      4th Phase – Fertilization

At this stage, sperm and oocytes are mixed in a petri dish and then brought together in the culture medium prepared in the laboratory and waited for fertilization. During this period, your doctor checks for signs of fertilization and monitors the growth of the embryos.

5th Phase – Placing The Fertilized Oocyte Into The Uterus

After fertilization, embryo development is monitored and transfer is made on the appropriate day. Transfer strategies vary depending on the number of oocytes obtained and the quality of the developing embryo. The number of embryos to be transferred is selected according to the quality and period of the developing embryos, taking into account the woman's age and previous unsuccessful in vitro fertilization attempts. Embryos are placed in the uterus with the help of a thin catheter. After placement is completed, individuals are given daily progesterone and HCG injections to strengthen and support the uterine lining and increase the likelihood of implantation.

      6th Phase – Pregnancy Test

A pregnancy test is performed in the 2nd week after embryo transfer to determine whether pregnancy has occurred. If the test is positive, an ultrasound is performed 10 days after the test to see the sac.

Things To Consider After Ivf Treatment

Sometimes implantation bleeding or spotting may occur after the transfer. Light spotting (pink or brown discharge) may be accompanied by mild cramping for 5 to 7 days after the transfer. In this case, inform your doctor, do not stop taking your medications, and take your pregnancy test on the day your doctor recommends.

It is very important to rest for a few days after the transfer. Bed rest is not generally recommended, but may be recommended in some cases. During this period, any activity that will strain the body should be avoided.

Recommended progesterone medications should be used regularly. Patient should eat healthy and avoid consuming foods such as raw meat and milk. Apart from these, individuals should try to avoid stress and encourage themselves to think positively.
