      Colectomy operation is the procedure of removing part or all of the large intestine. This surgery aims to treat early-stage colon cancer. It is also applied in Chron and Ulcerative colitis diseases.

      There are two method of Colectomy ; Laparoscopic Colectomy and Open Colectomy. This surgery, which involves cleaning the tumorous large intestine and surrounding lymph nodes, is performed under general anesthesia and takes between 1 and 5 hours.

      Who Are Eligible Candidates For Colectomy?

      If individuals have Chron disease, which has symptoms such as intestinal cramps, diarrhea, constipation, blood in stool and weight loss; If there is ulcerative colitis that causes discomfort such as inability to defecate, anemia and abdominal swelling, they are considered suitable for colectomy operation.

      However, the actual colectomy operation is performed on individuals diagnosed with suspicious tumors in the colon or colon cancer as a result of various examinations and colonoscopy.

      How Is The Colectomy Performed?

      Colectomy operation, which has two types: open and laparoscopic (closed), is performed to clean the tumor area in the colon and the surrounding lymph. This operation, performed under general anesthesia, takes between 1 and 5 hours.

      PREPARATION

      Firstly, it is crucial to have physical examination with surgeon. According to the physical examination and findings, wurgery will be planned. After the surgery is planned, there are some pre-operative tests and consultation for the surgery. The first step is learning patient’s anamnesis (medical record) and the following steps are Blood tests, ECG (Electrocardiography), Chest X-ray, computed tomography (CT Scan), ultrasound, Colonoscopy, MRI, PET-CT and anesthesiologist consultation will be done.
      According to the patient’s anamnesis, surgeon may ask additional tests and consultations.
      The purposes of these preoperative tests are to evaluate the patient's risk, understand how far the cancer has spread, and protect the patient's health.

      ANESTHESIA

      Colectomy operation is performed under general anesthesia which is numbing whole body and put to patients to sleep.

      COLECTOMY TECHNIQUE

      After the anesthesia is performed according to the colon cancer situation the method of surgery will be determined by the surgeon.
      In the open colectomy, an incision is made from the belly button to the lower abdomen. In this way, the colon and tumor area are accessed. Generally, the colon is cut 10 cm away from the tumor and at the same time the main vessels feeding the tumor are connected from its root. Depending on the condition of the cancer, the intestine larger than 10 cm is cut. This surgery may involve removing the lymph around the colon, except for the tumorous part of it, or removing the organs where the tumor has spread, such as the spleen and pancreas. There are two ways to complete this surgery. One is to create an anastomosis, which is created by sewing the two cut intestine ends together. Or, a part of the intestine is sewn outside the body and a colostomy bag is attached to the end.
      In the laparoscopic colectomy, it starts with 3 or 4 small incisions in the abdomen. Laparoscopic surgery instruments are inserted through the incisions. Laparoscopic colectomy is generally preferred in cases where the cancer has not spread much. They may then remove the diseased or damaged part of the colon and sew the two ends together (anastomosis) or create a stoma.

      Recovery From Colectomy In Turkey

      Patients will stay at the hospital at least for 1 week. After returning home, patients should dress as recommended by their doctor. If dissolvable stitches are used, there is no need to remove the stitches. If non-dissolvable stitches are used, the stitches should be removed within 10 to 21 days or more.

      After colon cancer surgery, patients are asked to pay attention to their nutrition. They are informed by their doctor whether they will start chemotherapy or another treatment. Patients are advised to move in a way that does not tire them out in order to prevent blood clot formation.

      If a colostomy bag is placed, the doctor explains how to keep the colostomy clean and how to change it. The doctor will tell you how long the colostomy will remain, but in some patients the colostomy must be used for life.

      Total recovery after surgery takes between 1 and 3 months.
