- What is Brachymetatarsia?
Brachymetatarsia is a condition where one of the metatarsal bones is shorter than normal. This usually affects the fourth toe, which looks shorter or lifted. The condition happens because the bone stops growing too early.

There are two surgical options; first one is single-stage lengthening (bone graft), in the single-stage lengthening method, the surgeon cuts the bone and inserts a bone graft to make the metatarsal longer. The graft can come from your own body, from a donor or from an artificial material. After placing the graft, the surgeon uses plates and screws to keep the bone stable while it heals. Second one is Gradual lenghtening (external fixator), in the gradual lenghtening method, a small external fixator device is attached to the bone with thin wires. You turn the device every day to slowly increase the lenght of the bone. The fixator stays on the foot for about 3 to 6 months until the new bone becomes strong and fully healed.
- What is the recovery time?
The recovery time depends on the surgical technique used. In the single-stage lengthening method, patients usually wear a cast for 5 to 6 weeks, then use a walking boot for another 4 weeks, and they can start wearing trainers at around 10 weeks, although swelling may continue for 6 to 12 months. In the external fixator method, patients wear a cast for about 6 weeks, make daily adjustments to the fixator for the first 3 weeks, and keep the fixator on for 3 to 6 months with full healing sometimes taking up to one year.
- When can I walk again?
For the first 6 weeks after surgery, patients cannot put any weight on the foot in either surgical method. After 6 weeks, they can slowly begin weight-bearing while using a protective boot. Patients who have an external fixator may still need to do partial weight-bearing even after the device is removed, depending on how well the bone has healed.
