      Toe Walking Treatment

      Toe walking is a walking pattern where children walk on the balls of their feet without letting their heels touch the ground. It’s normal for toddlers just learning to walk, and most children grow out of it by the age of 2. When toe walking continues beyond this age, it’s rarely linked to underlying medical conditions and is more commonly idiopathic, meaning there’s no clear cause. Sometimes, older children toe walk due to habit or tightness in their calf muscles. Treatments like bracing or casting can help by gently stretching the calf muscles and encouraging a more natural walking style.

      The calf is made up of two key muscles. Gastrocnemius, the larger, more prominent muscle forming the visible bulge beneath the skin, and soleus, a smaller, flat muscle located beneath the gastrocnemius. These muscles join at the base of the calf to form the Achilles tendon, which connects to the calcaneus (heel bone). When the calf muscles contract, they pull on the Achilles tendon, which in turn moves the heel.

      Some children who toe walk may have a shorter calf muscle-tendon combination, either present from birth or developed over time. This can make it difficult for them to place their heels on the ground while walking. However, most children who toe walk have a muscle-tendon combination long enough to walk flat-footed when reminded.

      Who are the eligible candidates for Toe Walking Treatment?

The surgery is performed under general anaesthesia and the duration varies according to the extent of your child’s particular case.

      Preparation for the Surgery

      A physical exam will first be conducted where the doctor will first observe your child’s walk before the child even realizes they’re being watched, to avoid the so-called "doctor walk," where they try extra hard to walk properly. The doctor will then ask your child to demonstrate their typical toe walking as well as their best attempt at walking flat-footed. These observations go beyond just checking for toe walking, they help assess the smoothness and coordination of the walk, providing important insights into your child’s neurological function.
      During the physical exam, the doctor will check for foot abnormalities, differences between the left and right foot, tight calf muscles, hip and knee range of motion, and any skin or structural issues in the lower body and back. The doctor will assess the flexibility of the calf muscles by asking your child to move their feet and ankles in different directions. Additionally, they’ll check the range of motion in the hips and knees.

The doctor will check for muscle tightness in the arms and legs, assess major muscle strength, test reflexes using a small hammer or fingertip, and evaluate sensation in the limbs. Since idiopathic toe walking is diagnosed when no other problems are identified, additional tests like X-rays, MRIs, or nerve studies are typically unnecessary unless the exam suggests another condition.

      Types of Anaesthesia

      Achilles tendon lengthening surgery is usually carried out under general anesthesia, which means the patient will be asleep and unaware of the procedure. In some cases, regional anesthesia, such as a nerve block, may be used in combination with general anesthesia to help control pain after the surgery, ensuring a more comfortable recovery.

Lastly, gastrocnemius recession procedure which focuses on the gastrocnemius muscle, lengthening and loosening the fibers to improve ankle movement. After completing the procedure, the surgeon will close the incisions, and your child will begin the recovery process, typically involving physical therapy.
      Tendon transfers are a surgical approach used for children with recurrent toe walking, aiming to correct foot position and restore proper gait. Tendons like the tibialis anterior, tibialis posterior, or flexor hallucis longus are repositioned to enhance foot function. The surgery involves making an incision near the affected tendon, isolating it, and then carefully detaching it from its original attachment point. The tendon is then repositioned to a more effective location to control foot positioning and restore a more normal walking pattern. Two common tendon transfers for toe walking are the tibialis posterior tendon transfer, which moves the tendon to the anterior tibialis to help keep the foot in a dorsiflexion position, and the flexor hallucis longus tendon transfer, which helps control toe position and improve overall gait mechanics. After the tendon is secured in its new position with sutures, the wound is closed.

      • Pain management will be prescribed, including ibuprofen, Tylenol, or stronger medications if necessary. Make sure to follow the doctor’s directions for administering these medications carefully.
      • Nausea can be common as the anesthesia wears off and may last for up to 24 hours. Stick to clear liquids and easily digestible foods during this time and avoid greasy or heavy meals. If vomiting occurs, wait 30 to 60 minutes before trying any food or liquids again.
      • For the first 48 hours, keep the leg elevated with pillows to reduce swelling, and encourage your child to wiggle their toes to help with circulation.
      • Once the cast is removed, your child will wear a boot for two weeks and begin partial weight bearing (PWB).
